This reporting system is designed to help us learn from incidents, that are made in the practices to allow us as a team to grow our knowledge of safety. These reports are completely anonymous to avoid any and all confrontation between the team members, and to improve all safety in practices. Safety is very important to us as a team, and we must be able to see the mistakes or accidents that others make.
